Implantology

Implantations

For implantations we use local anaesthetics – for example, the traditional dental injections which numb the area. We cut the mucous membrane and, as directed by the manufacturer, prepare locations for the screws by drilling. After this we check the impression whilst screwing into the jawbone. Finally we close the wound by stitching.
After a week or 10 days we remove the stitches and leave the screws to settle in - to ossify. This takes about 4–6 months. Naturally we provide a temporary bridge or temporary removable prosthesis for this period so that the patient should not be without teeth even for one day.
After the ossification process we clean around the screws, separating them from the gums (by a technique similar to biscuit-cutting!) and we start to prepare the denture.
This is illustrated in the images below.

Case analysis

Stages in building and anchoring bridges

Two implants followed osteoplasty and an operation to lift the maxillary sinus. After 4 months immediately following the screwing-in of gum-formers; implants.

Artificial stumps for making impressions.
The huge advantage is that no grinding of the teeth is required.
Taking an impression is fast and painless!

During osteoplasty we implant artificial osteoplastic materials. These create bones in a way in which a whole screw become a part of the bone.
In the other case the osteoplastic implantation material is much more visible.
Black arrows: the edges of the osteoplastic material.
Red lines: those parts of implants inside the osteoplastic material (and not in the air-space of the oral cavity.)

A further case from 2007. Treatment of a totally toothless patient with implants - after lifting both sides of the maxillary sinus. The green line shows osteoplastic material implanted through a bone-window. Finally we see the patient’s complete plastic-ceramic bridge.

Stages in the implantation process:

  1. where needed - ossifying or lifting maxillary sinus
  2. implantation of material (if possible we combine stages 1 and 2)
  3. 7–10 days after stage 2 - removal of stitches
  4. after some 4–6 months - separation of gum and implant. (During this time implant has had time to settle and ossify)
  5. after 4–5 days – begin to prepare denture - which takes 3–4 weeks, depending on type.

Naturally, progress should be checked frequently.

We use the: Straumann SLA and CAMLOG Screw type Implant systems.
